The Department of intensive cardiology of prematurity and infants with congenital heart disease
The Department of intensive cardiology of prematurity and infants with congenital heart disease, established in 1999, is a structural unit of the Bourakovsky Institute for Cardiac Surgery, a part of the Federal State Budget Scientific Organization "Bakoulev Scientiific Center for Cardiovascular Surgery" (Bakoulev SCCVS). Since 2001 Tumanyan Margarita Rollandovna, MD, Professor, Honored Doctor of the Russian Federation, pediatric cardiologist of highest category, the head of training course for physicians in the specialty "Pediatric Cardiology" of the Department of Cardiovascular Surgery and Interventional Cardiology of A.I. Yevdokimov Moscow State University of Medicine and Dentistry, has been the head of the Department. Deputy head of the Department is the pediatric cardiologist of the highest category Chechnya Vera Vladimirovna. Clinical work in the Department is performed by doctors, specialists of the highest category – pediatric cardiologists and cardiovascular surgeons, an, full of attention to small patients: MD Svobodov Andrey Andreevich, CMS Levchenko Elena Grigorievna, CMS Anderson Alexey Georgievich, CMS Kotova Anna Nikolaevna, CMS Kotov Sergey Aleksandrovich, CMS Gulasaryan Ruben Sergeevich, Butrim Yulia Valeryevna, Chvamaniya Alexander Alexandrovich, Makarenko Mariya Vladimirovna, Zubkova Svetlana Aleksandrovna. Besides there is a highly qualified nursing team in the Department, who cares for children with great love at all stages of treatment.
The Department has 24 beds, some of which are provided for a separate stay of the child and mother. On the territory of the Department there is an intensive care unit, which has three beds equipped with newest equipment for artificial ventilation for extremely severe patients. The Department carries out work on complex diagnostics of congenital heart and vascular diseases, the conducting tactics for children with CHD and cardiac rhythm disorders, therapeutic training and surgical treatment of newborns and children of the first year of life with complex congenital cardiac pathology, using all the techniques available at Bakoulev SCCVS. Therapeutic tactics depends on the nature of the congenital pathology and the severity of concomitant diseases. A qualified approach to treatment in future allows such children not only develop according to age norms but also avoid disability. Significant part in the Department practice is given to advisory work, and mostly to polyclinic appointments of patients in Bakoulev SCCVS as well as in medical institutions of Moscow, Moscow region and other regions of Russia. Department employees usually take part in consultations which are regularly held directly in the maternity hospitals of Moscow and the Moscow region. Modern technologies such as teleconferences are used to provide consultations for newborns from remote regions of Russia allowing not only identify a child's condition and determine indications for cardiac surgery, but also give qualified recommendations for arrangements to transport a child to Bakoulev SCVVS. With help of telecommunications the Department staff has an opportunity to conduct lecture courses and seminars on topical issues of pediatric cardiology in remote as well.
In the scope of congenital heart defects at which corrective surgeries are successfully performed, a group of "critical congenital heart defects" of the neonatal period should be noted, namely, transposition of the main vessels, pulmonary atresia, all anatomical variants of obstructive lesions of the arch and aortic valve, aggregate arterial trunk, stenosis of pulmonary artery and aorta valves, left heart hypoplasia syndrome, total anomalous drainage of pulmonary veins, etc. In addition to the "critical" heart defects of the neonatal period, surgical treatment of small patients with the following diagnosed congenital heart defects: tetralogy of Fallot, double vascular withdrawal from the right ventricle, heart tumors, single ventricles of the heart, Ebstein's anomaly, atrioventricular communication, pathology of valvular valves, defects of interventricular and interatrial septa, etc., is also performed with minimal risks. The Department developed unique operations for a number of congenital heart defects (pulmonary atresia, heart tumors, coarctation and aortic arch breaks, etc.), the use of which significantly improved the results of surgical treatment.
Over the past 10 years, the Department staff performed more than 7000 out-patient consultations in the maternity hospitals of Moscow and Moscow region - almost 40% of newborns were transferred for surgical treatment to Bakoulev CVVS because of urgent indications. Over the same years, more than 6000 children of the first year of life were operated on the basis of the Department: almost 60% among them were infants of the first three months of life, more than 30% of them - premature babies and children with significant body weight deficit.
