Department staff
Department of interactive pathology surgical treatment is one of the largest divisions of A.N. Bakulev Center for cardiovascular surgery of the Russian Ministry of Health, the leader in the number of operations and diagnostic procedures performed.
The department provides the full range of diagnostic aids, conservative therapy, cardiac surgery and minimally invasive interventions for almost any heart pathology, including acquired and congenital heart diseases, coronary artery disease and heart rhythm disturbances, brachiocephalic (carotid) arteries diseases.
The staff of the department consists of young cardiovascular surgeons, cardiologists as well as specialists with immense experience in cardiac surgery. The staff includes one Academician of the RAS, 3 PhDs, 13 MDs. Scientific researchers are engaged in the study of topical issues of clinical medicine, publishing scientific papers in leading journals in Russia, as well as in foreign editions. At present, the department is headed by Academician L.A. Bockeria and Professor S.A. Donakanyan, PhD.

Main directions of clinical work

The department performs operations of any complexity for various congenital and acquired heart diseases. Including:

  • “Maze 3B”, electrophysiological methods of treatment, implantation of devices that replace the heart functions;
  • surgery for chronic heart failure, including implantation of artificial heart ventricles, as well as heart transplantation in patients with end-stage heart failure;
  • operations for combined lesions of the heart valves and coronary arteries, surgery for congenital heart defects (ASD, VSD, Ebstein anomaly);
  • in case of the coronary arteries lesions, coronary artery disease and coronary Kawasaki syndrome: surgical myocardial revascularization (coronary and mammary bypass grafting), endovascular balloon angioplasty and coronary stenting; transmyocardial laser myocardial revascularization using CO2 laser in combination with intramyocardial injection of patient's bone marrow stem cells;
  • operations for brachiocephalic arteries pathology;
  • surgical repair in case of pulmonary artery thromboembolia, other interventional procedures and surgical operations.
Surgical interventions for ischemic heart disease:
  • coronary artery bypass grafting on a beating heart under cardiopulmonary bypass;
  • minimally invasive myocardial revascularization;
  • transmyocardial laser revascularization;
  • the left ventricle geometric reconstruction in case of left ventricular aneurysm;
  • coronary arteries stenting.
Surgical interventions for acquired heart diseases:
  • heart valves repair;
  • heart valves replacement;
  • TAVI (transcatheter aortic valve implantation);
  • Mitral Clip implantation;
  • Heart tumors treatment.
Surgical interventions for cardiomyopathies:
  • hypertrophic cardiomyopathy repair;
  • heart transplantation;   
  • artificial heart ventricle implantation.
Surgical interventions for arrhythmias:
  • Maze procedure;
  • removal of previously infected electrodes;
  • pacemaker implantation;
  • cardioverter-defibrillator implantation;
  • electrophysiological examination;
  • radiofrequency/cryoablation of arrhythmogenic zones.
Combined operations:
  • coronary artery bypass grafting and carotid endarterectomy;
  • coronary artery bypass grafting and the left ventricle geometric reconstruction;
  • coronary artery bypass grafting and epicardial radiofrequency/cryoablation in case of heart rhythm disturbances;
  • coronary artery bypass grafting and valvular pathology repair;
  • valvular pathology repair and Maze procedure.
Surgical interventions for congenital heart diseases:
  • total repair of aortic coarctation;
  • atrial/ventricular septal defect plasty;
  • occluder implantation for atrial/ventricular septal defect;
  • atrioventricular canal repair;
  • repair of pulmonary artery stenosis under cardiopulmonary bypass;
  • Ebstein anomaly repair;
  • Valsalva sinus aneurysm repair;
  • removal of the anterior mediastinum cyst;
  • transluminal balloon angioplasty of aortic valvular stenosis;
  • repair of coronary-pulmonary fistulas and anomalies of the coronary arteries.
Surgical interventions for vascular pathology:
  • carotid endarterectomy;
  • resection of brachiocephalic arteries tortuosity;
  • brachiocephalic arteries stenting;
  • renal arteries stenting;
  • stenting of the upper and lower extremities arteries.

Statistics

Since its opening, more than 30,000 patients have been examined and treated and a number of unique operations have been performed in the department. About 5,000 patients have been operated on-pump, about 15,000 have undergone endovascular diagnostic and treatment procedures - these are coronary angiography, stenting, electrophysiological examinations, radiofrequency ablation, implantation of pacemakers, defibrillators, resynchronization devices.
In 2022, 525 heart operations under cardiopulmonary bypass and 301 interventions on a beating heart were performed. The number of endovascular procedures was more than 1000.

Conditions of staying in the department

The Department of interactive pathology surgical treatment is located on the fifth floor of the Center main building (135, Rublevskoye shosse). There is a cozy lobby to welcome patients.
The department has excellent conditions for accommodating patients, with a capacity of 50 beds. Budget patients are accommodated in general wards for 3-4 people with all amenities. If you want a more comfortable stay, there are one- and two-room separate wards. This type of service is commercial.
There is a common room to accommodate relatives on the territory of the Center.
DIPST also has an intensive care ward (ICW) with 8 beds - for the treatment of patients on the first day after transfer from the intensive care unit (ICU), as well as for severe patients who need constant monitoring.
On weekday mornings, patients are rehabilitated in the department: in the first postoperative days patients are taught to breathe correctly, to do special exercises, and from the second or third day patients already walk around the department and join physical exercises activities in the hall.
Such an active approach allows to avoid nosocomial pneumonias, which are extremely difficult to treat due to the fact that their pathogens are not susceptible to most antibiotics. The department has its own canteen for patients, bed-bound patients are fed in the wards.
For diagnostic procedures, the department has an echocardiography room, a functional diagnostics room with modern equipment for stress testing, 24-hour ECG monitoring, a unique ultrasound machine that allows you to perform a three-dimensional ultrasound examination of the heart in real time.
