DEPARTMENT OF SURGERY OF OLDER CHILDREN WITH CHD (OLDER THAN 3 YEARS OLD) (CHD)

Podzolkov Vladimir Petrovich Podzolkov Vladimir Petrovich

Deputy Director for science

Head of Department of surgery of older children with CHD (older than 3 years old)

PhD in Medicine, professor, academician of the RAS

What we treat
Department staff
CHD Department has the largest experience in Russia in treatment of CHD patients (more than 20,000 heart and great vessels operations, 10,000 of them under cardiopulmonary bypass). Heart operations under cardiopulmonary bypass as well as the repair of most congenital heart diseases were performed for the first time in Russia in this department.
Annually, the department provides care to more than 1,000 patients with congenital heart diseases.

Head of CHD Department is the Awardee of the State Prize of the USSR, Honored Scientist of the Russian Federation, Academician of the RAS, PhD in Medicine, Professor
VLADIMIR PETROVICH PODZOLKOV

Main directions of clinical work

The department conducts examination and treatment of children older than 3 years and adult patients with congenital heart diseases of any complexity with a total mortality of 0.2% and during operations under cardiopulmonary bypass - 0.7% (data of 2022).
The department performs a wide range of surgical and endovascular interventions, allowing to provide care for both simple and the most complex CHDs. Various types of artificial pulmonary artery trunk are used to repair complete and corrected transposition of the great arteries, aorta and pulmonary artery origin from the right and left heart ventricles, and pulmonary atresia.
The extensive experience has been accumulated in the surgical treatment of patients using the Fontan method (hemodynamic correction) for atresia of the tricuspid and mitral valves, the only ventricle of the heart, for criss-cross heart and other defects that are not subject to anatomical correction (over 550 operations).
The operations of reconstruction and replacement of heart valves with congenital pathology are commonly performed (over 1500 operations).
The issue of CHD surgical treatment in adults is being successfully dealt with. Simultaneously with the repair of the defect in CAD, coronary artery bypass grafting is performed; in case of heart rhythm disturbances, they are corrected (the oldest patient was 71 years of age).
To correct the long-term postoperative complications (recanalization of defects, dysfunction of the valves, the right ventricle of the heart), re-operations are performed, including re-replacement of heart valves.

Types of operations developed and implemented in the department

Surgical treatment using the Fontan method (hemodynamic correction) for atresia of the tricuspid and mitral valves, the only ventricle of the heart, for criss-cross heart and other defects that are not subject to anatomical correction is performed (over 550 operations).
Heart valve replacement surgeries are commonly performed in congenital pathology both in adults and children.
One of the main directions of the department’s surgical activity is the heart valves reconstructive surgery. Various methods of mitral and tricuspid valve reconstruction have been introduced into clinical practice, including re-operations. Various reconstructive operations and the Ozaki technique are used for the aortic valve congenital pathology.
The issue of CHD surgical treatment in adult patients is being successfully dealt with. In stages or simultaneously with the repair of congenital heart disease, coronary artery bypass grafting or stenting of the coronary arteries can be performed in case of coronary artery disease. Particular attention is paid to the correction of rhythm disturbances, which in most cases can be eliminated simultaneously with congenital heart disease.
One of the leading directions of the department’s medical and scientific activity is the surgical treatment of right ventricular dysfunction, which develops in a number of patients after CHD total repair. The developed methods of pulmonary valve replacement, elimination of residual defects, tricuspid insufficiency, and heart rhythm disturbances can prevent fatal complications and significantly improve the quality of life of patients with right ventricular dysfunction.

Conditions of staying in the department

The bed capacity of the department is 24 beds. There are general wards, as well as comfortable single wards designed for patients to stay with one of the parents. The department is equipped with modern medical equipment.
