DEPARTMENT OF ISCHEMIC HEART DISEASE SURGICAL TREATMENT (IHDST)
Main directions of clinical work
- Surgical treatment of patients with ischemic heart disease
- Surgical treatment of the coronary circulation acute disorders: unstable angina and myocardial infarction
- Surgical treatment of complicated forms of ischemic heart disease: post-infarction heart aneurysm, ischemic cardiomyopathy, interventricular septum post-infarction rupture
- Surgical treatment of patients with combined atherosclerotic lesions of the coronary artery and blood vessels supplying the brain (brachiocephalic arteries)
- Surgical treatment of patients with multifocal atherosclerosis (combined lesion of several vascular pools)
- Surgical treatment of patients with angina recurrence after previously performed coronary artery bypass grafting
- Surgical treatment of IHD patients with additional operational risk factors (elderly and aged patients, patients with concomitant diseases, patients with diffuse lesions of the coronary arteries)
Types of operations developed and implemented in the department
- Coronary artery bypass grafting operations both on-pump and off-pump
- The left ventricle geometric reconstruction associated with post-infarction aneurysm and ischemic cardiomyopathy in combination with coronary artery bypass grafting
- Simultaneous and staged operations of carotid endarterectomy and coronary artery bypass grafting in patients with combined lesions of the coronary and carotid arteries
- Repair of intrathoracic lesions of the brachiocephalic arteries in combination with coronary artery bypass grafting
- The left ventricle geometric reconstruction in combination with plasty or mitral valve replacement and coronary artery bypass grafting in ischemic mitral insufficiency
- The left ventricle geometric reconstruction in combination with plasty of post-infarction ventricular septal defect using the original technique developed in the department
Statistics
Over the past ten years, the department annually performs 350-450 surgical interventions for various forms of ischemic disease, 40-60 percutaneous coronary interventions and other transcatheter procedures. Hospital mortality is generally 3-4%, 0.5-1.5% for coronary artery bypass surgery, 5-6% for the repair of post-infarction aneurysms and is constantly decreasing, despite more severe patient population.
