DEPARTMENT OF RECONSTRUCTIVE SURGERY OF HEART VALVES AND CORONARY ARTERIES (RSHVCA)
Main directions of clinical work
- reconstructive and valve-sparing operations on the heart;
- surgery for combined lesions of heart valves, coronary arteries, aortic branches;
- valve-sparing and reconstructive operations on the ascending aorta and arch;
- cardio-oncology;
- surgical treatment of infective endocarditis, especially its complicated forms with tissue destruction and abscess formation;
- minimally invasive surgery for acquired heart defects.
Types of operations developed and implemented in the department
- reconstructive surgery on the mitral, tricuspid and aortic valves;
- coronary artery bypass grafting, as well as in combination with operations on the heart valves;
- single-valve and multi-valve replacement with mechanical and biological prostheses;
- replacement of the ascending aorta and aortic valve in case of aortic aneurysms;
- valve-sparing operations for aneurysms of the ascending aorta (David procedure, Florida Sleeve procedure);
- Ozaki procedure – restoration of the aortic valve from the patient's own pericardium;
- aortic valve replacement with aortic root dilatation in case of narrow aortic annulus fibrosus;
- replacement and reconstruction of heart valves in infective endocarditis, restoration of destroyed paravalvular structures of the heart;
- re-interventions after previously performed heart surgeries;
- operations for benign and malignant tumors of the heart (myxomas, fibroelastomas, rhabdomyomas, metastases of other tumors);
- minimally invasive operations on valves, as well as through ministernotomy, minithoracotomy, using video endoscopic methods;
- stenting of coronary and peripheral vessels is performed in the department, as well as TAVI - transcatheter aortic valve implantation.
The department demonstrates excellent results in surgery for combined lesions of heart valves and coronary arteries. Due to the technique developed here, the mortality in this extremely severe pathology is minimized and does not differ from the mortality in isolated lesions of the heart valves.
Professor I.I. Skopin, head of the Department made a great contribution to the development and application of combined operations for lesions of the heart valves and coronary arteries, various methods of aortic root reconstruction, heart valve bioprosthetics, including the introduction of the Ross operation, as well as surgery for acquired heart diseases, tricuspid valve replacement with a mitral allograft.
The department continues to develop a method of optimal mini-access for valvular pathology of acquired heart defects, including repeated interventions. Much attention is paid to reconstructive interventions aimed at reverse remodeling of the left ventricle.
